WHO cautions against consuming non-sugar sweeteners for weight cotrol

Geneva. The World Health Organization (WHO) has released a new guideline on non-sugar sweeteners (NSS), which recommends against the use of NSS to control body weight or reduce the risk of noncommunicable diseases (NCDs).

The recommendation is based on the findings of a systematic review of the available evidence which suggests that use of NSS does not confer any long-term benefit in reducing body fat in adults or children. Results of the review also suggest that there may be potential undesirable effects from long-term use of NSS, such as an increased risk of type 2 diabetes, cardiovascular diseases, and mortality in adults.

“Replacing free sugars with NSS does not help with weight control in the long term. People need to consider other ways to reduce free sugars intake, such as consuming food with naturally occurring sugars, like fruit, or unsweetened food and beverages,” says Francesco Branca, WHO Director for Nutrition and Food Safety. “NSS are not essential dietary factors and have no nutritional value. People should reduce the sweetness of the diet altogether, starting early in life, to improve their health.”

The recommendation applies to all people except individuals with pre-existing diabetes and includes all synthetic and naturally occurring or modified non-nutritive sweeteners that are not classified as sugars found in manufactured foods and beverages, or sold on their own to be added to foods and beverages by consumers. Common NSS include acesulfame K, aspartame, advantame, cyclamates, neotame, saccharin, sucralose, stevia and stevia derivatives.

The recommendation does not apply to personal care and hygiene products containing NSS, such as toothpaste, skin cream, and medications, or to low-calorie sugars and sugar alcohols (polyols), which are sugars or sugar derivatives containing calories and are therefore not considered NSS.

Because the link observed in the evidence between NSS and disease outcomes might be confounded by baseline characteristics of study participants and complicated patterns of NSS use, the recommendation has been assessed as conditional, following WHO processes for developing guidelines. This signals that policy decisions based on this recommendation may require substantive discussion in specific country contexts, linked for example to the extent of consumption in different age groups.

The WHO guideline on NSS is part of a suite of existing and forthcoming guidelines on healthy diets that aim to establish lifelong healthy eating habits, improve dietary quality and decrease the risk of NCDs worldwide.

Pancreatic cancer vaccine shows promise in small initial trial

An mRNA vaccine prevented tumor recurrence after surgery in eight of 16 patients, but the therapy still needs to be validated in larger trials

Pancreatic cancer is one of the deadliest forms of cancer, with very few effective treatments. But messenger RNA (mRNA) vaccines—famous for their ability to prevent COVID—are starting to show some promise against the lethal cancer. In a recent early-stage trial, half of pancreatic cancer patients who received a personalized mRNA cancer vaccine after surgery did not have a recurrence of the tumor a year and a half later. The trial, which was described in a study published on Wednesday in Nature, was small—with just 16 patients—and it will need to be replicated in larger studies.

“I am very supportive of the findings,” says Drew Weissman, director of vaccine research and director of the Institute for RNA Innovation at the University of Pennsylvania, who is a pioneer of mRNA vaccines but was not involved in the new paper. He adds that “it is not a definitive proof-for-use study. Larger studies are needed to determine effectiveness.”

Pancreatic ductal adenocarcinoma, the most common type of pancreatic cancer, has a mortality rate of 88 percent. It is the third leading cause of death from cancer in the U.S. and is becoming more common. Surgery is the main form of treatment, but the cancer has a 90 percent recurrence rate at seven to nine months. Chemotherapy is only partially effective at delaying recurrence. Other treatments, such as immunotherapy, are mostly ineffective.

Pancreatic cancer often goes undetected until its later stages, when it is harder to treat. One reason it is so sneaky is that it generates relatively few of a type of surface proteins, called neoantigens, that mark it as foreign and trigger an immune response. Scientists had noticed that people who survived pancreatic cancer had a stronger response to these neoantigens from T cells, a type of immune cell.

In the new study, Vinod Balachandran, an assistant attending surgeon at Memorial Sloan Kettering Cancer Center in New York City, and his colleagues targeted pancreatic cancer patients’ own tumor neoantigens using mRNA technology—the same technology used to create the remarkably successful COVID vaccines. The experimental vaccines used by Balachandran and his colleagues were produced by BioNTech, a company that developed one of the COVID vaccines with Pfizer. The researchers vaccinated a total of 16 patients. After surgically removing the tumors, they treated the patients with mRNA vaccines tailored to each person’s specific cancer, as well as an adjuvant, a substance that increases the effects of vaccines. Fifteen of the participants were also treated with chemotherapy.

Eight of the 16 patients generated a strong T cell response to the vaccines. At a median follow-up time of 18 months after the treatment, these individuals had longer survival, without a recurrence of their cancer.

The study was small and involved only white patients. And the therapy—which is expensive—does not work for everyone with pancreatic cancer. Nevertheless, experts say it is a promising development for a disease with such limited treatment options.

Thai election: Who is Pita Limjaroenrat, the Move Forward Party leader

“Today is a new day, and hopefully it is full of bright sunshine and hope,” 42-year-old Pita Limjaroenrat told a room packed with reporters and flashing cameras, hours after it had become clear that he could well be Thailand’s next prime minister.

Mr Pita’s party, Move Forward, which in Thai evokes leaps of change, has swept to a surprising and stunning victory. Voters rejected nearly a decade of army-backed rule, handing more seats and votes to Move Forward than any other party. That is is no mean achievement in a country that has experienced at least a dozen successful coups.

“The sentiment of the era has changed. And it was the right timing,” Mr Pita said, speaking at the party’s headquarters in Bangkok, where it had won 32 of the city’s 33 seats.

The business of forming a government is still weeks away. It can also get messy, given that Thailand’s military still has the power to play kingmaker. But the win has put the spotlight on Mr Pita and his party, who have built a strong following among young voters disillusioned by years of military rule and hungry for change.

Mr Pita started his political career when he was elected to parliament in 2019 as a member of the Future Forward Party. Founded by Thanathorn Juangroongruangkit, a billionaire and staunch critic of the army, the party performed well in the 2019 election, shaking up Thai politics with its demand for change.

But Future Forward was forced to disband, the following year in the wake of contentious allegations. And Mr Thanathorn was disqualified as an MP. Move Forward was formed soon after as its successor and named Mr Pita its new leader.

Thousands of young people took to the streets in Thailand that year after the ban, demanding amendments to the constitution, a new election and an end to the harassment of rights activists and state critics. That desire for change and many of those very issues drove Move Forward’s appeal in this election, with some of the protest leaders from 2020 running as candidates.

Mr Pita – once called a “rising star” of the Thai parliament because of the critical speeches he made as an opposition MP – also gained popularity with his party’s bold promises to break the military’s political influence and reform laws relating to the monarchy.

Move Forward will continue to push forward lèse-majesté reforms, Mr Pita reiterated on Monday. “We will pass that in this parliament. And we will use the parliament to make sure that it is a comprehensive discussion with maturity, with transparency, in how we should move forward in the relationship between the monarchy and the masses,” he added.

Mr Pita was born to a wealthy Thai family with political connections. His father was an adviser in the agriculture ministry and his uncle was an aide of former ousted PM Thaksin Shinawatra.

He has said he developed an interest in politics during his high school days in New Zealand.

“I got shipped to the middle of nowhere in New Zealand and there were three [TV] channels back then. Either you watch Australian soap operas, or you watch the debates in parliament,” he told the Thai YouTube programme Aim Hour in February this year.

He graduated with a bachelor’s degree in finance in Bangkok’s Thammasat University, a master’s in public policy from Harvard University and an MBA from the Massachusetts Institute of Technology.

But he started his career in business, first taking over the running of his late father’s rice bran oil company and later as an executive director of ride-hailing company Grab.He was married to Chutima Teenpanart, a Thai actress-model, but the couple divorced in 2019. He is now a single dad of seven-year-old Pipim, whom he has brought to Move Forward’s rallies. (with reports from BBC)

“… We are confident that with our friendly relations and strong people-to-people links, we shall be able to find a mutually satisfactory solution that will take into account the need to provide maximum protection and access to justice for all our nationals working in the country,” ayon dito.

“We assure everyone that the DMW will work with all of its partners to mitigate the impact of this recent development to our Kuwait-bound workers,” dagdag pa ng DMW.
